There are different challenges in inner city areas compared to semi-rural/rural areas.  The main difference being a greater concentration of population that are more ethnically diverse in inner city areas.  This also brings the constraints of cultural issues and the apparent distrust of health services from some cultures; a good example is the comparatively low uptake of covid-19 vaccines and the expensive campaigns to persuade people that the vaccine is beneficial to them, their families, and the wider population.
